COVID-19 Protocols & Requirements:

Please review the following COVID-19 related expectations of event participants: - I will be fully vaccinated against COVID-19 by the time of the event. - I will follow strict safety and health protocols while in attendance of the event, such as mask-wearing, social distancing, frequent disinfection and limited room capacities per the latest CDC guidelines. - I will self-monitor my health prior to and throughout the event and will NOT come to the event if I am feeling unwell or experiencing any symptoms associated with COVID-19. - After the completion of the event, if I test positive for COVID-19 within 14 days I will notify The Forum of my positive test result.
