Menu Home > COVID-19 Protocols & Requirements COVID-19 Protocols & Requirements Full Name Email Address COVID-19 Protocols & Requirements:Please review the following COVID-19 related expectations of event participants: - I will be fully vaccinated against COVID-19 by the time of the event. - I will follow strict safety and health protocols while in attendance of the event, such as mask-wearing, social distancing, frequent disinfection and limited room capacities per the latest CDC guidelines. - I will self-monitor my health prior to and throughout the event and will NOT come to the event if I am feeling unwell or experiencing any symptoms associated with COVID-19. - After the completion of the event, if I test positive for COVID-19 within 14 days I will notify The Forum of my positive test result. Agreement(Required) I understand and agree to comply with the COVID-19 related protocols and requirements outlined by the Forum. I also understand that failure to provide proof of vaccination by November 5th will result in an automatic cancellation of my in-person event registration and may use this credit for future Forum events within a year. COVID-19 Vaccination Information:All participants must be fully vaccinated by November 5th and provide proof of vaccination prior to traveling to Boston in order to attend the in-person event.
